Our Benefits Guide highlights the various options and will help you make an informed decision about your health care.
The goal of this guide is to allow you to make well-informed decisions that can help you reduce your out-of-pocket health care costs.

MEDICAL PLANS (Anthem & Kaiser)
During Open Enrollment, you chose between our plans, provided by Anthem Blue Cross or Kaiser Permanente.

Kaiser information: Pages 8, 12 & 13.
Anthem information: Pages 9, 10, 13 & 14.

Types of Plans
Pure Project offers two plan types:
HMO (Health Maintenance Organization)
Less flexibility, little access to out-of-network treatment.
Require a Primary Care Physician (PCP). Each family member can select a different PCP. You can also change your PCP by calling the phone number on the medical ID card.
If you need a specialist, your PCP will determine the specialty and make a referral.
For emergencies, you can go to the nearest facility. However, you must contact your PCP within 24 hours.
For hospitalizations, ask your PCP for the best location to go. If you are away from home, reach out to your PCP to receive authorization.
PPO (Preferred Provider Organization)
Do not require a PCP.
More flexibility to choose where to seek treatment.
Some plans require a deductible to be met before coverage begins. Once you're deductible is met, you still need to pay a % of the cost of treatment. Cost per year fluctuates more than HMO plans.
We highly recommend using in-network facilities and pharmacies to save money.
Health Plan Lingo
PCP (Primary Care Physician):
Some plans ask you to choose a specific doctor to oversee your care. They will recommend specialist visits, prescription drugs, and treatments.
Coinsurance:
This is your percentage of the cost each time you get care, once the deductible has been paid
Deductible:
The amount of money you have to pay before your plan starts to help cover expenses
Copay:
This is a set amount you pay for covered services, such as doctor visits.
Out-Of-Pocket:
This is the maximum amount you pay out of your pocket for covered health services each year.

We offer two plan types: PPO & DHMO.
PPO: You can visit any dentist. We give employees the option to choose a High or Low option, depending on how much or the percentage you will pay for each service covered.
DHMO (Managed Dental): We assign a dentist for you, and you enjoy negotiated discounts for services covered.

VSP offers significant out-of-pocket savings by visiting In-Network doctors. They are also one of the largest private practice provider networks.

Includes STD, LTD, Voluntary Life and AD&D Benefits (UNUM)
Pure Project covers 100% of the cost of the benefits below:
Short Term Disability (STD): Covers injuries that keep you from working. This insurance covers 70% of your monthly income while you recover.
Long Term Disability (LTD): Covers injuries that keep you from working for a long time. Covers 66.6% of your monthly income while you recover.
Life & AD&D (Accidental Death & Dismemberment) Insurance: Provides money to your family if you die or are diagnosed with a terminal illness.
Note: Additional coverage for Voluntary Life & AD&D is available to employees, spouses, and their children via Guardian.
Our Employee Assistance Program (EAP) is provided through Guardian & UNUM. Pure Project covers 100% of the cost of this benefit.
EAP provides guidance for personal issues that you might be facing and other concerns that affect your life.
The program is designed to help employees with support, counseling, and improvement of quality of life and work.
Examples:
Talk to a professional counselor about stress & anxiety, family problems, work conflicts, relationship issues, etc.
Talk to a specialist about financial health and debt management, child or elder care, legal counsel, and even how to reduce your medical bills!
