OPEN ENROLLMENT 2024 & 2025
OPEN ENROLLMENT is the only time of year when you may make changes to your enrollment status in your benefits unless there is a Qualifying Event (marriage, childbirth, for example).
THE ENROLLMENT WINDOW OPENS ON MONDAY JULY 15TH AND WILL CLOSE FRIDAY, JULY 26TH.
IMPORTANT INFORMATION:
ALL ELIGIBLE TEAM MEMBERS (WORKING 30+ HOURS PER WEEK) ARE REQUIRED TO GO THROUGH OPEN ENROLLMENT - EVEN THE ONES THAT JUST RECENTLY ENROLLED IN BENEFITS.
IF YOU ARE NOT INTERESTED IN BENEFITS, YOU STILL NEED TO GO INTO THE PORTAL AND DECLINE COVERAGE - THIS IS A REQUIREMENT BY LAW.
NOTE THAT WHATEVER YOU ELECT AT THIS TIME IS GOOD FROM 8/01/2024 - 7/31/2025 UNLESS YOU EXPERIENCE A QUALIFYING EVENT.
ALL ELIGIBLE TEAM MEMBERS WILL HAVE THE OPPORTUNITY TO MAKE THE FOLLOWING ELECTIONS/CHANGES:
ADD OR DROP DEPENDENT COVERAGE.
CHANGE/OPT-OUT FROM PLANS.
ENROLL IN PLANS IF YOU HAVE NOT PREVIOUSLY ENROLLED.

How to start the Open Enrollment Process?
Go to your Paycom Portal;
Under Benefits, select "2024 Benefits Enrollment" or click your Notifications.
Review initial instructions and click "Start Enrollment";
Enter your personal information and any dependents or beneficiaries.
After reading each benefit plan, choose your coverage, then elect either to enroll or decline
To complete enrollment, click “Finalize,” then “Sign and Submit”
Important: Please make sure to click save and continue on every screen. If you are DECLINING coverage you need to go in and check “Don’t want this benefit?” then Save and Continue.
INFORMATION FOR 2024-2025 ENROLLMENT:
Pure Project contributes monthly 80% of your Medical Base Plan and $25 for Dental and Vision.
All plans will be effective 08/01/2024.
If you do not pick a plan by 07/26, you will not be enrolled in benefits!
If you select an HMO Plan, make sure to add your current Primary Care Physician (PCP) information. If you don't know their information or don't have a PCP, reach out to HR or Liliana Queiroz (our benefit broker).
HOW TO FIND YOUR PROVIDER ID? PRIMARY CARE PHYSISCIAN (PCP) - HMO PLANS ONLY
If you are not changing carriers, you should already have a PCP assigned to you. Take a look at your Medical ID card to find their information.
If you have a new carrier, you can choose a PCP that is near to your home/work:
Kaiser - Find Care page. Add your zip code and filter "Primary Care Physicians accepting new patients"
Anthem - Find Care page. Search as a Guest > Select Medical Plan, State, Medical (Employer Sponsored), select the plan you're choosing - Select or Priority HMO
